Sin Double Strike ?
If you land a double strike on a lead or offhand attack does the skill affect or damage bonus trigger twice, or would you just get the normal skill damage/affect and then an extra stab in as well. Actually, Lead and Offhand attacks don't have a chance to double strike. Double striking only applies to Dual attacks and auto-attacking.

I think double strikes only works on dual attacks and normal attacks, never on lead or offhand attacks.

hmm, unless your Assassin has 4 arms you can't have double lead/off hand attacks

Dual attacks always hit twice, hence the name dual attack. Double Strikes only occur while auto attacking.
